Quick Facts

  • Two half-day clinic sessions weekly plus SNF coverage on other days
  • 4 – 6 patients per clinic session, 5 – 10 SNF patients daily
  • Outpatient clinic and skilled nursing facility setting
  • SNF panel of approximately 150 residents with bi-monthly visits
  • New admissions require 2 – 3 hours, routine monthly visits about 1 hour
  • Combined panel of 50 outpatients and under 200 SNF residents
  • Hospital privileges required
